Who Can Apply for a UMID Card?

You are eligible to apply if you:

  • Are an active member of SSS or GSIS with complete and updated records.
  • Have not been issued a UMID card before (unless applying for renewal or replacement).
  • Meet your agency’s specific membership requirements (posted contributions, updated records, etc.).

UMID Card Requirements (2025)

UMID Application Form – A fully accomplished UMID application form is required.

Primary Valid ID (if available)
One of the following:

  • Driver’s License
  • Passport
  • Social Security 2D Barcode ID Card
  • PhilID Card
  • Seaman’s Book
  • Voter’s ID
  • Alien Certificate of Registration
  • Firearm Registration
  • NBI Clearance
  • Postal ID

Two Secondary IDs (if no primary ID)
If you don’t have any of the primary IDs, you can submit two secondary IDs:

  • Both must have your signature.
  • At least one must have your photo.

  • School ID
  • Company ID
  • Senior Citizen ID
  • TIN ID
  • Pag-IBIG Member’s Data Form
  • PhilHealth Member’s Data Record
  • Birth Certificate
  • Marriage Contract
  • Police Clearance
  • School Transcript of Records (TOR)

Membership Requirements:

  • GSIS Members: Active membership with updated records.
  • SSS Members: At least one posted contribution.
  • Pag-IBIG Members: Active membership.
  • PhilHealth Members: Active account.

Important Notes:

  • If you applied for an SSS number online since December 10, 2020, you may not need a separate UMID application form (SSS advisory).
  • Production and delivery delays are possible.
  • First-time issuance is free; replacements have a fee.

Step-by-Step UMID Application Process

  1. Prepare Your Documents

Ensure all IDs match your membership records. Correct any discrepancies before applying.

Through your issuing agency’s website (SSS or GSIS). Walk-ins may be limited.

  • Submit your form and requirements.
  • Have your photo, fingerprints, and signature captured.

Processing typically takes 4–6 weeks, depending on your location.

  1. Activate Your UMID Card

If it includes ATM functionality (e.g., SSS disbursement account), follow the activation steps provided.

Renewal or Replacement of UMID
